About the Role
SynergenX is hiring a part-time telemedicine provider (FNP or PA) to deliver patient care across Indiana. This is a fully remote, W-2 role focused on evidence-based hormone optimization, wellness care, and preventive health.
You’ll evaluate patients through telehealth and support long-term relationships while practicing in a collaborative environment.
Responsibilities
- Provide telehealth assessments, diagnosis, and treatment planning for patients in Indiana
- Deliver personalized care plans, including hormone replacement therapy (HRT) and medical weight management
- Monitor patient progress and adjust treatment plans as clinically indicated
- Maintain accurate clinical documentation and follow established care protocols
Requirements
- Active Indiana license as an FNP or PA
- Current Indiana DEA and CSR registrations
- Strong commitment to patient-centered, evidence-based care
- Comfort working in a remote, telemedicine-based clinical setting
