About the Role
Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PTA) to support patient-centered care in a skilled nursing facility. This full-time role offers a Monday–Friday schedule with flexibility based on clinic needs.
Responsibilities
- Assist the physical therapist with patient evaluations and treatment plans
- Provide hands-on therapy support to improve mobility, strength, and function
- Document patient progress according to facility and clinical standards
- Help maintain a supportive, collaborative environment for patients and the care team
Requirements
- Current licensure as a Physical Therapy Assistant (PTA)
- Experience in skilled nursing or a clinical setting preferred
- Strong communication skills and a patient-first mindset
