About the Role
UC Health is seeking a full-time Ambulatory Telehealth Registered Nurse (RN) to serve as a Care Coordinator for patients in an outpatient setting. This role blends patient-facing nursing care with care coordination and administrative responsibilities, with about 60% of your time spent directly with patients.
Responsibilities
- Provide telehealth nursing support to patients with complex and chronic conditions during outpatient visits
- Collaborate with ambulatory physician clinics to support care needs before, during, and after appointments
- Develop, implement, and help execute individualized care plans
- Educate patients on diagnoses, treatment plans, and self-management strategies
- Coordinate with interdisciplinary team members to improve care quality and patient experience
- Ensure seamless communication and follow-through across care settings
Requirements
- Active Registered Nurse (RN) license
- Experience in ambulatory care and/or care coordination preferred
- Strong communication and patient education skills
- Ability to manage both direct patient care and administrative care coordination tasks
- Comfort working in a telehealth environment
