About the Role
Nest Health is building a family-first, value-based care model that brings medical, behavioral, and social support within reach for high-risk families. As a Medical Assistant & Family Advocate in Flagstaff, you’ll help deliver coordinated in-home and virtual care by supporting clinical workflows and connecting families to the resources they need.
Responsibilities
- Assist with patient intake, vitals, and basic clinical support to keep visits running smoothly
- Support scheduling, follow-ups, and documentation in coordination with the care team
- Help coordinate vaccinations, labs, and routine care needs as directed
- Provide family advocacy by identifying barriers and guiding patients to social services and care navigation
- Support acute visit readiness and chronic care coordination, including transitions of care
- Maintain professionalism, confidentiality, and a patient-centered approach at every interaction
Requirements
- Experience working as a Medical Assistant (or closely related clinical support role)
- Comfort working with families and supporting patients across medical and non-medical needs
- Strong communication skills and attention to detail for documentation and follow-up
- Ability to support in-home and/or virtual care activities as needed
- Commitment to compassionate, culturally responsive care
