About the Role
Ennoble Care is a mobile primary care, palliative care, and hospice provider serving patients across multiple states. As a Patient Care Coordinator in our Marlton, New Jersey office, you’ll help ensure patients receive coordinated, compassionate support from a team that knows and understands them.
Responsibilities
- Coordinate patient care activities to support continuity across services
- Work with the care team to help schedule and manage patient needs
- Serve as a point of contact to support patients and families throughout the care journey
- Assist with program coordination such as remote patient monitoring and chronic care management
- Maintain accurate, organized documentation to support care delivery
Requirements
- Experience in a patient coordination, care coordination, or related healthcare support role
- Strong communication skills and a patient-first mindset
- Comfort working in a fast-paced, care-focused environment
- Ability to collaborate effectively with clinicians and other team members
