About the Role
Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant to provide PRN support in a skilled nursing facility. This role is ideal for a flexible schedule with realistic daily shift planning to help maintain work–life balance.
Responsibilities
- Deliver one-on-one patient care with minimal group/concurrent coverage (<1%)
- Assist the physical therapy team in implementing treatment plans
- Support patients with therapy exercises and mobility activities as directed
- Maintain a patient-centered, compassionate approach in a skilled nursing setting
Requirements
- Current licensure as a Physical Therapy Assistant
- Experience working in or supporting skilled nursing/rehab environments (preferred)
- Strong communication and teamwork skills
