About the Role
Join Enhabit Home Health & Hospice as a Registered Nurse (RN) and help lead clinical care for patients in their homes. This Clinical Team Leader role supports a collaborative, purpose-driven team focused on delivering high-quality home health services.
Responsibilities
- Coordinate and lead day-to-day clinical activities for the home health team
- Provide skilled nursing care in accordance with individualized patient plans
- Support care coordination by partnering with patients, families, and interdisciplinary team members
- Help ensure documentation and clinical processes meet organizational and regulatory standards
Requirements
- Current RN licensure in Idaho
- Home health or community-based care experience preferred
- Strong clinical judgment, communication, and leadership skills
- Ability to travel within the assigned service area
