About the Role
Join a next-generation virtual specialty clinic delivering preventive cardiometabolic care to older adults. This fully remote Advanced Practice Provider role (Nurse Practitioner or Physician Assistant) focuses on improving hypertension management and lowering cardiovascular risk for members with elevated risk of atherosclerotic cardiovascular disease.
Responsibilities
- Provide cardiometabolic specialty care through a virtual clinic model
- Assess and manage patients with hypertension and cardiovascular risk factors
- Develop and maintain preventive care plans to support long-term outcomes
- Collaborate with the care team to optimize treatment and follow-up
- Use bilingual communication skills to support Spanish/English-speaking members
Requirements
- Active NP or PA license (clinician type: APRN or Physician Assistant)
- Experience in cardiometabolic care, primary care, or chronic disease management preferred
- Bilingual Spanish/English proficiency
- Ability to work Monday–Friday 8:30am–5:00pm MST/PST (EST/CST also considered)
