About the Role
Cadence is seeking a Bilingual/Spanish Speaking Clinical Navigator (Medical Assistant) to support Medicare patients enrolled in Cadence care programs. In this remote role, you’ll help deliver proactive chronic care outreach and follow-through that keeps patients engaged and supported.
Responsibilities
- Reach out to patients to support ongoing chronic care needs and program participation
- Provide patient-centered guidance and coordinate next steps based on patient status
- Monitor for risks and ensure appropriate follow-up actions are completed
- Document outreach and outcomes accurately in clinical systems
- Collaborate with care teams to support continuity of care
Requirements
- Experience working as a Medical Assistant in a clinical setting
- Fluency in Spanish and English (bilingual communication required)
- Comfort working with virtual patient engagement workflows
- Strong communication, empathy, and attention to detail
