About the Role
Moments Hospice is hiring a Hospice Social Worker to support patients and families during end-of-life care. In this role, you’ll help ensure comfort, provide emotional support, and connect individuals with the resources they need—making a meaningful difference during some of life’s most challenging moments.
We also understand that commuting shouldn’t be stressful. Our compensation package includes a fleet car benefit option with gas and insurance covered, with a vehicle available for both business and personal use at minimal cost to you.
Responsibilities
- Assess patient and family psychosocial needs and develop supportive care plans
- Provide counseling, crisis support, and guidance to patients and caregivers
- Coordinate care with the interdisciplinary hospice team
- Help connect families to community resources and benefit programs
- Maintain documentation and ensure timely, accurate updates to care plans
Requirements
- Relevant social work experience, preferably in hospice, palliative, or related settings
- Strong communication and empathy for patients and families
- Ability to manage responsibilities with reasonable caseload expectations
- Comfort collaborating with a multidisciplinary team
