About the Role
Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist (OT) to join our team in a skilled nursing facility setting. This full-time role supports patient-centered care with a collaborative, therapist-forward approach.
Responsibilities
- Provide individualized occupational therapy services to patients
- Deliver one-on-one therapy sessions and support patient goals
- Collaborate with the care team to support treatment plans and progress
- Maintain accurate documentation in accordance with facility and regulatory requirements
Requirements
- Active state licensure as an Occupational Therapist (OT)
- Experience in skilled nursing or similar clinical settings preferred
- Strong communication skills and a patient-focused mindset
- Ability to work Monday–Friday with flexible scheduling
