About the Role
Giving Home Health Care has been supporting individuals affected by health conditions connected to their work in nuclear facilities for the Department of Energy since 2012. We provide personalized in-home care and help our patients receive the support they need with dignity and consistency. As a Patient Coordinator, you’ll serve as a key point of contact across our care team—helping patients, caregivers, and internal departments stay aligned.
Responsibilities
- Act as a liaison between patients, caregivers, and internal teams to support smooth care coordination
- Help ensure clear communication and timely updates throughout the patient journey
- Coordinate information flow between Giving Home Health Care and relevant departments
- Support a patient-first experience by addressing questions and helping resolve issues
