About the Role
Innovate Rehab and Wellness is seeking a licensed PRN Physical Therapist to provide high-quality care in a skilled nursing facility. This role offers flexible, as-needed shifts with realistic daily scheduling designed to support work–life balance.
Responsibilities
- Deliver skilled physical therapy services in a skilled nursing facility setting
- Provide patient-centered treatment with a strong one-on-one care focus
- Collaborate with the care team to support safe, effective outcomes for residents
Requirements
- Current state licensure as a Physical Therapist
- Experience or comfort working in skilled nursing or similar care environments
- Ability to deliver compassionate, individualized therapy to patients
