About the Role
Innovate Rehab and Wellness is seeking a licensed Speech-Language Pathologist (SLP) for PRN, as-needed coverage in a skilled nursing facility. This role offers flexible scheduling and the opportunity to make a meaningful impact on patients who need speech and swallowing support.
Responsibilities
- Provide speech and language evaluations and treatment plans for patients in the skilled nursing facility
- Deliver therapy services that support communication, cognition, and swallowing goals
- Collaborate with the interdisciplinary care team to support patient outcomes
- Document progress and maintain accurate clinical records in accordance with facility and regulatory requirements
Requirements
- Current state licensure as a Speech-Language Pathologist
- Experience working in skilled nursing or similar healthcare settings preferred
- Strong clinical documentation and communication skills
- Ability to adapt to a flexible, PRN schedule
