About the Role
Ennoble Care is seeking a full-time Care Coordinator with experience supporting Durable Medical Equipment (DME). In this role, you’ll coordinate key steps that help patients receive the equipment and services they need, while working closely with a trusted home-based care team.
Responsibilities
- Coordinate DME referrals and support the end-to-end process from request through delivery
- Collaborate with internal care teams to ensure patient needs are documented and addressed
- Communicate with patients, caregivers, and relevant providers to keep care moving smoothly
- Track and manage coordination tasks to support timely equipment fulfillment
- Maintain accurate records and follow organizational workflows
Requirements
- Experience in care coordination, DME coordination, or a related healthcare coordination role
- Strong communication skills with a patient-centered approach
- Ability to manage multiple coordination tasks while maintaining attention to detail
- Comfort working in a fast-paced, mobile care environment
