About the Role
Ennoble Care is seeking a full-time Care Coordinator to support medical documentation efforts for our home-based primary, palliative, and hospice patients. This role is based in our Marlton, New Jersey office and helps ensure our clinical teams have accurate, timely documentation to support coordinated care.
Responsibilities
- Coordinate and support completion of medical documentation for patient care workflows
- Review documentation needs and help ensure records are complete and up to date
- Work closely with clinical staff to support continuity of care
- Maintain organized documentation processes that align with care standards and program requirements
- Assist with administrative follow-through to support timely care delivery
Requirements
- Experience in care coordination and/or medical documentation support
- Strong attention to detail and comfort working with health information
- Ability to collaborate effectively with a multidisciplinary team
- Professional communication skills and a patient-centered mindset
- Willingness to work onsite from our Marlton, NJ office
