About the Role
The STOP Case Manager Supervisor leads a team of STOP Case Managers providing direct support to program participants. You will oversee intake, initial assessments, placement into community-based services, transportation coordination, and ongoing case planning and case management. You will also collaborate closely with the Division of Adult Parole Operations (DAPO) to ensure referrals move forward efficiently and accurately.
Responsibilities
- Supervise and support STOP Case Managers delivering participant casework services
- Ensure referrals are administratively processed within three (3) business days of receipt
- Assign participants to Case Managers and maintain caseload ratios (not to exceed 1:__ as applicable)
- Oversee development and execution of participant case plans, including initial placement in community-based services
- Coordinate participant transportation and facilitate access to required services
- Serve as a key point of contact for coordination and communication with DAPO
Requirements
- Experience supervising staff or leading a case management team
- Strong knowledge of case management practices, intake, assessment, and placement processes
- Ability to manage administrative workflows and ensure timely processing of referrals
- Excellent communication skills and ability to partner with external stakeholders (including DAPO)
- Commitment to participant-centered service delivery and program compliance
