About the Role
Circle the City is a growing non-profit community health organization providing holistic, high-quality care to people experiencing homelessness across Arizona. The Float Nurse Practitioner role supports services across multiple program sites, helping ensure patients receive timely, compassionate clinical care.
Our model includes medical respite care in two 50-bed centers, psychiatric consultation options, community-based services through our assertive community treatment team, and an upcoming mobile clinic to expand outreach.
Responsibilities
- Provide clinical assessment, diagnosis, and treatment within scope of practice across assigned sites
- Deliver patient-centered care for individuals experiencing homelessness, including those with complex medical and behavioral health needs
- Collaborate with interdisciplinary teams to coordinate care plans and follow-up
- Support continuity of care across respite, outreach, and other service settings
- Document clinical encounters and maintain accurate patient records
Requirements
- Current Nurse Practitioner licensure in the state of Arizona
- Experience working in primary care, urgent care, inpatient, or community health settings preferred
- Strong clinical judgment and the ability to manage a wide range of patient needs
- Compassionate, trauma-informed approach to care
- Ability to travel between program locations as needed
