About the Role
Ennoble Care is hiring a Full-time Hospice Intake Nurse (LPN) to support our Philadelphia, PA market on a hybrid schedule. In this role, you’ll help ensure patients and families receive timely, compassionate hospice intake and care coordination—often as part of care delivered in the home.
Responsibilities
- Complete hospice intake assessments and gather required clinical information
- Coordinate with the interdisciplinary hospice team to support smooth admissions
- Serve as a key point of contact for patients, families, and referral sources
- Document intake details accurately and maintain compliance with care and reporting requirements
- Support ongoing transitions of care for patients with serious illness
Requirements
- Current LPN license in good standing
- Experience in hospice, palliative care, or home health preferred
- Strong communication skills with patients, families, and care teams
- Comfort with documentation and care coordination in a fast-paced environment
- Ability to work a hybrid schedule supporting the Philadelphia market
