About the Role
Ennoble Care is hiring a Full-time Intake Nurse (LPN) to support our Freehold, NJ market on a hybrid schedule. In this role, you will help ensure patients receive timely, high-quality intake and onboarding as part of our mobile primary care, palliative care, and hospice services.
Responsibilities
- Complete patient intake activities and coordinate next steps for care
- Collect and document relevant clinical information accurately and efficiently
- Support care teams by communicating intake needs and updates
- Assist with scheduling and workflow to ensure smooth patient transitions
- Maintain compliance with applicable clinical and documentation standards
Requirements
- Active Licensed Practical Nurse (LPN) license
- Strong clinical documentation and attention to detail
- Ability to work effectively in a hybrid environment and manage time well
- Compassionate, patient-centered approach
