About the Role
Hospice of San Joaquin is a not-for-profit hospice with 40 years of experience serving patients across San Joaquin and Stanislaus Counties. We’re looking for a dynamic, self-motivated LVN who can thrive in a fast-paced environment and help us deliver compassionate, patient-centered care—especially during weekend coverage every other weekend.
Responsibilities
- Provide direct nursing support to hospice patients in accordance with care plans and established protocols
- Assist with symptom management and monitor patient condition, reporting changes to the care team
- Support caregivers by reinforcing care instructions and maintaining communication with the hospice team
- Document care activities accurately and in a timely manner
Requirements
- Active Licensed Vocational Nurse (LVN) license
- Ability to work effectively in a fast-paced, mission-driven setting
- Strong communication skills and attention to detail for documentation
