About the Role
We’re seeking a Nurse Care Manager Lead (RN) to manage care for patients with acute and chronic conditions across multiple client programs. In this role, you’ll serve as a clinical subject matter expert while mentoring and guiding team members to ensure high-quality, compliant care management.
Responsibilities
- Assess and evaluate potential care management patients in line with client contractual requirements and applicable state regulations
- Partner with physicians and ancillary providers to confirm appropriate treatment plans and identify education opportunities
- Guide members to the right benefit information and support resources
- Provide ongoing patient and provider education, including communication to ensure care remains appropriate
- Recommend and implement care adjustments when needed; evaluate adherence and reassess patient engagement to drive necessary changes
Requirements
- Registered Nurse (RN) license
- Experience in case management and/or care coordination for patients with acute and chronic conditions
- Strong clinical assessment, communication, and collaboration skills
- Ability to mentor and support team members as a subject matter expert
