About the Role

Responsibilities
- Provide patient-centered wound care assessments and treatment plans
- Deliver care in-person at partner facilities
- Work with a team model supported by administrative resources and an intuitive EMR
- Maintain accurate clinical documentation to support continuity of care
Requirements
- Active Nurse Practitioner (NP) license
- Clinical experience in wound care or related specialty preferred
- Strong communication skills and a patient-focused approach
- Ability to provide care across assigned facilities
