Nurse Practitioner or Physician Assistant (NP/PA) – Primary Care – Spanish Speaking – Hybrid/Onsite

About the Role

Astrana Health is looking for a compassionate Primary Care NP/PA to join our outpatient team at Gateway Medical Center in the Anaheim area. You’ll provide patient-centered care with an emphasis on prevention, chronic disease management, and value-based approaches.

Gateway Medical Center is a multi-specialty practice serving the community since 1984, with clinic locations in Anaheim, Anaheim Hills, and Santa Ana, California. Spanish-speaking clinicians and providers with geriatric experience are strongly preferred.

Responsibilities

• Deliver comprehensive primary care to patients across the lifespan
• Focus on preventive services and evidence-based health maintenance
• Manage chronic conditions and coordinate follow-up care
• Support value-based care goals in an outpatient setting
• Collaborate with care teams to improve patient outcomes

Requirements

• Active NP or PA licensure and ability to work in California
• Experience in primary care and/or outpatient medicine
• Strong communication skills and a patient-first mindset
• Spanish language proficiency strongly preferred
• Geriatric experience strongly preferred
