About the Role
Moments Hospice is seeking an on-call Registered Nurse (RN) to help coordinate and direct hospice services for patients in the community. This schedule follows a 7 on/7 off rotation, with on-call support available for case managers so you’re never working in isolation.
Responsibilities
- Organize, coordinate, and direct hospice care in accordance with patient needs and care plans
- Provide experienced clinical nursing support with a focus on community health education
- Support case managers through timely on-call communication and guidance
- Ensure hospice services are delivered with clear expectations and reasonable caseloads
Requirements
- Current Registered Nurse (RN) license
- Experience in nursing with hospice or community health experience preferred
- Strong communication skills and the ability to educate and support patients and families
- Ability to respond effectively in an on-call capacity
