About the Role
Innovate Rehab and Wellness is seeking a licensed Physical Therapist to provide PRN coverage in a skilled nursing facility. This role is designed for flexible, as-needed shifts with realistic daily scheduling to support work–life balance.
Responsibilities
- Deliver hands-on physical therapy care in a skilled nursing facility setting
- Provide individualized treatment with an emphasis on one-on-one patient attention
- Collaborate with the care team to support each patient’s plan of care
Requirements
- Current Physical Therapist license (state licensure)
- Experience working with patients in post-acute/skilled nursing settings preferred
- Ability to adapt to a flexible PRN schedule
