About the Role
Innovate Rehab and Wellness is seeking a licensed PRN Physical Therapist to provide high-quality care in a Skilled Nursing Facility. This role is ideal for a clinician who values flexibility while delivering meaningful, individualized therapy.
Responsibilities
- Provide skilled physical therapy services to patients in a skilled nursing setting
- Deliver primarily one-on-one care to support patient progress
- Work flexible, as-needed shifts with realistic daily scheduling
- Partner with the facility team to support therapy goals and continuity of care
Requirements
- Current state licensure as a Physical Therapist
- Experience working in skilled nursing or geriatrics is preferred
- Strong clinical judgment and commitment to patient-centered care
