About the Role
Innovate Rehab and Wellness is seeking a licensed PRN Physical Therapist to provide high-quality care in a skilled nursing facility. This role is ideal for a therapist who values autonomy, flexible scheduling, and meaningful one-on-one patient time.
Responsibilities
- Deliver physical therapy services tailored to each patient’s needs in a skilled nursing setting
- Provide primarily one-on-one care, with minimal group/concurrent treatment
- Support patient progress through goal-focused treatment plans and ongoing reassessment
- Collaborate with the care team to help patients maintain mobility, function, and independence
Requirements
- Current state licensure as a Physical Therapist
- Experience working in skilled nursing or similar settings preferred
- Strong clinical judgment, communication skills, and a patient-first mindset
- Ability to work flexible, as-needed shifts
