About the Role
INTEGRIS Health Baptist Home Health is seeking a full-time Registered Nurse (RN) Care Manager for day shift coverage in Oklahoma City, Oklahoma. In this role, you’ll coordinate individualized patient care and help patients and families navigate home health services to support healthier outcomes.
Responsibilities
- Complete skilled nursing assessments and develop patient-specific interventions
- Coordinate ongoing care plans and communicate updates with the care team
- Support continuity of care by ensuring appropriate services are delivered in the home setting
- Contribute to patient-centered care through education and care coordination
Requirements
- Current RN licensure in the state of Oklahoma
- Experience in home health, case management, or skilled nursing preferred
- Strong clinical assessment and care coordination skills
- Compassionate, patient-focused communication
