About the Role
Ennoble Care is seeking a full-time, experienced RPM (Remote Patient Monitoring) Care Coordinator to support patients through home-based primary, palliative, and hospice services. This role is based out of our Hackensack, New Jersey office and helps ensure patients receive coordinated, high-quality care throughout their health journey.
Responsibilities
- Coordinate remote patient monitoring workflows to support ongoing patient care
- Collaborate with clinical teams to track patient status and care needs
- Assist in managing patient enrollment and monitoring processes
- Help ensure care plans are communicated clearly across the care team
- Support documentation and follow-up to maintain continuity of care
Requirements
- Experience as a Care Coordinator and/or in RPM (Remote Patient Monitoring) support
- Strong communication and organization skills
- Ability to work onsite from the Hackensack, NJ office
- Commitment to patient-centered care and teamwork
