About the Role
Join ELMHS’s Rehabilitation Department as a Rehab Evaluator-Entry in our Patient Education Program. In this role, you’ll help clients identify educational needs and support learning through tailored plans and hands-on instruction.
Responsibilities
- Assess clients’ educational needs and determine appropriate learning goals
- Develop individualized learning plans based on client needs and progress
- Provide patient education through group sessions and one-on-one instruction
- Support documentation of educational goals, participation, and outcomes
- Collaborate with the rehabilitation team to reinforce care plans and continuity of education
Requirements
- Entry-level experience or strong interest in patient education, rehabilitation, or behavioral health
- Ability to communicate clearly and compassionately with diverse clients
- Comfort working with clients in a structured program setting
- Basic understanding of learning plans and goal tracking
