About the Role
Habitat Health empowers older adults to experience more good days through the Program of All-Inclusive Care for the Elderly (PACE). We deliver coordinated clinical and social support both in our centers and in participants’ homes, helping individuals maintain independence with confidence and community.
As we expand our scalable PACE model to meet increasing needs, we’re seeking a Senior Medical Director to strengthen physician excellence and support high-quality, mission-driven care across our programs.
Responsibilities
- Lead initiatives that promote clinical quality, consistency, and best practices across physician teams
- Support physician performance through engagement, coaching, and evidence-based guidance
- Collaborate with clinical operations and cross-functional teams to improve care delivery and outcomes
- Help develop and implement standards for care coordination, documentation, and quality metrics
- Contribute to continuous improvement efforts that align with PACE requirements and organizational goals
Requirements
- Medical degree and active clinical licensure (state requirements as applicable)
- Experience in physician leadership, clinical quality, or value-based care
- Strong understanding of care coordination and interdisciplinary care models
- Ability to influence and partner effectively with clinicians, administrators, and program leaders
- Commitment to serving older adults and advancing patient-centered care
