About the Role
We’re seeking a motivated Transplant Pharmacy Liaison to support patients and clinicians through our specialty pharmacy management services. In this role, you’ll work independently while building trusted relationships with patients, healthcare providers, and internal teams—always with a strong focus on patient quality, service, and follow-through.
Responsibilities
- Serve as a primary point of contact for patients and clinicians regarding transplant pharmacy needs
- Coordinate support across multiple systems and internal tools to keep care plans on track
- Communicate clearly and empathetically by phone and in person
- Maintain accurate, detail-oriented documentation and administrative organization
- Collaborate with a close-knit team to ensure consistent, high-quality patient experiences
